What is Service Coordinator Role?
At Align Services Coordination, our Service Coordinators are dedicated to providing comprehensive support to ensure our clients receive the care and services they need to live independently and comfortably at home.
- Facilitating Initial Service Plan (ISP) & Eligibility
Our Service Coordinators assist in developing a personalized Initial Service Plan (ISP) for each client, reviewing eligibility for waiver programs that provide the necessary support and resources. - Regular Visits
Our commitment to client care includes monthly face-to-face visits and quarterly in-home visits. These interactions allow us to monitor the client’s well-being, adjust services as needed, and ensure ongoing satisfaction with service delivery. - Crisis Response
In times of emergency, our Service Coordinators are quick to respond, addressing and resolving any service-related issues promptly to maintain continuous, quality care for our clients. - Advocacy
We strongly advocate for our clients’ rights, ensuring they receive personalized and timely services tailored to their specific needs. This includes assisting with Medicaid Recertifications to maintain their eligibility for necessary services. - Coordination
Our Service Coordinators oversee the organized and efficient delivery of all services, enhancing the client’s quality of life while preserving their independence within their home.
